The ACE Advanced Health & Fitness Specialist Certification is designed for experienced personal trainers who want to expand their clientele beyond the restrictions of the "apparently healthy" category. The new manual and certification focus on those diseases and injuries most frequently encountered among clients in health and fitness facilities, from diabetes and low-back pain to lipid disorders and post-orthopedic rehabilitation.
ACE brought together 24 of the most sought-after experts in sports medicine and fitness, each writing on the focus on their own research or professional practice. In some cases, medical doctors and researchers were asked to explain the etiology and epidemiology of a disease or injury, while subject matter experts with a background in personal training focused on providing practical guidelines and sample exercises. The result is an informative reference text that also serves as a practical day-to-day guide for working with some of the most commonly seen special populations.
This manual is divided into five parts, the first of which provides an Introduction to this ACE certification program. The book begins by defining the scope of practice for an ACE-certified Advanced Health & Fitness Specialist (ACE-AHFS) and providing general guidelines for working with clients with health challenges. The introductory chapters also cover communication strategies and behavior change, professional relationships and business strategies and nutritional considerations for an active lifestyle. A theme that is introduced in this part of the manual and revisited throughout the text is the importance of establishing a quality referral network and knowing when and how to refer clients.
Part Two covers the Major Cardiovascular and Pulmonary Diseases and Disorders. This section begins with two chapters on coronary artery disease and blood lipid disorders. The first of these two chapters provides exercise recommendations for various manifestations of coronary artery disease, while the blood lipid disorders chapter provides a 10-step protocol that begins with evaluating the client's current status and includes ideas for partnering with the client's healthcare team. In addition to providing training guidelines, the chapter on hypertension examines the body's response to common hypertensive medications. The final chapter in this section covers asthma, including diagnostic testing and criteria and exercise guidelines.
Metabolic Diseases and Disorders are the focus of Part Three. The three chapters included in this section cover overweight and obesity, the metabolic syndrome and diabetes mellitus. The overweight and obesity chapter is broad in scope, covering everything from the biology of these conditions and the treatment of obesity to surgical interventions and biomechanical considerations specific to these populations. The chapter on the metabolic syndrome offers guidelines on the treatment and prevention of this common condition, as well as specific programming and progression guidelines. The diabetes mellitus chapter covers type 1, type 2, and gestational diabetes, explaining the benefits and risks of exercise for these populations and then offering guidelines for maximizing the benefits and managing the risks.
With eight chapters, Part Four, which covers Musculoskeletal Disorders, is the largest section of the manual. This section begins with two chapters on posture and movement and mobility, gait and balance. The first of these chapters offers an overview of postural assessment and a series of movement screens, and then provides information on restorative exercises for postural compensation and sample exercises. The second chapter explains the keys to proper gait and balance, discusses core function assessments, and offers a five-step programming protocol. The arthritis chapter that follows walks the reader through the normal course of symptoms in an arthritic joint and offers programming guidelines for the prevention and treatment of the condition. The next chapter in this section covers osteoporosis and osteopenia and includes information on bone's response to physical activity and exercise programming guidelines and progressions for various populations. The next three chapters work together, beginning with the principles of post-orthopedic rehabilitation and then looking specifically at injuries of the lower and upper extremities. After discussing topics such as concepts of healing and rehabilitation protocols, these chapters delve into specific injuries, including ACL injury, shin splints, rotator cuff pathology and shoulder instability. The final chapter in this section covers low-back pain and includes the five stages of building the ultimate back.
Part Five, the final section of the manual, offers Considerations for Specialized Population Groups, including older adults, youth and pre- and postnatal women. These three chapters offer training guidelines and precautions specific to each population. For example, the chapter on older adults discusses balance and gait issues, while the youth chapter focuses on fundamental movement skills and exercise program leadership issues. The pre- and post-natal exercise chapter covers everything from physiological changes associated with pregnancy and nutritional considerations for pregnant women to the benefits and risks of exercise following pregnancy and biomechanical considerations for the lactating mother.
Case studies are included in each chapter in Parts Two through Five, giving the exam candidate an example of how to apply what's been learned in a very practical way.
